Health Payment Terms:

A $25 deposit is required to reserve your appointment date and time. Your deposit is non-refundable, but it is applied toward your balance for services/products.

 

If you change your date and time, it must be done at least 24 hours in advance and receive a confirmation and approval from me, or you will forfeit your deposit, and your appointment will be canceled.
